Filing the N-400
You can find the N-400 and information about it at those links.
The form costs $640 plus a biometrics fee of $85, for a total of $725. If you need a fee waiver, the I-912 is is available for those experiencing financial hardship, living below 150 percent of the poverty line, and/or receiving means-tested benefit(s). You must attach this form to the N-400 when you file it if you want your fee waived.

If your income is between 150 and 200 percent of the poverty line, you may be eligible for a reduced fee, but not a fee waiver. If this applies to you, fill out the I-942. Please note that you will still have to pay $320 and the full $85 biometrics fee, so be prepared to do so.
You will also have to attach documents--if they are in another language, you must translate them into English. Find a complete list here. All applicants will at least have to include the following with their form:
-
A photocopy of your Legal Permanent Resident card
-
Check, money order, I-912/I-942, or G-1450 (to pay via credit card)
If you plan to hire your own lawyer to help you, fill out and attach form G-28 as well.
After this, a biometrics test and background check will be scheduled. Keep in mind that your picture from these tests may be placed on your Certificate of Naturalization. Next comes the interview.
